I started treatment at Form Dec 2011 after an injury to my leg and wrist. At this time, Washington Hospital referred me to a doctor in the area who I was not happy with, as he did not give me options. I wanted to heal quickly and efficiently, I did a search and found Form. I have been seen there since Dec. by Dr. Besh and Dr. J, who have helped me to a quick and relatively pain free recovery. They offered me options and have provided outstanding treatment. The staff at Form is also extremely nice and very helpful. I would recommend this office to anyone. |
